Part 1, Thoracic Pain That Doesn't Respond to Extension


Hi everyone, here's another patient I just saw that we can go through together.

Patient is a 42-year-old female yoga instructor with the onset of intermittent symmetrical mid-low thoracic pain about one year ago after roughhousing with her young boys. Twisting R is stiff when pain is elevated. Symptoms are worsening over time. Worse: twisting R, mornings, teaching yoga, standing with a forward lean. No red flags. No imaging. Sleep is limited due to pain.

THORACIC ROM: Baseline signs/ symptoms: 0/10 sitting at rest.

  • Flexion: no loss, feels good
  • Extension: min loss,
  • Rotation R: 70, ERP central TH
  • Rotation L: 70, ERP L scap

Sciatic neural tension: + L, much more pull compared to R

LE strength: 5/5 throughout bilat

LE reflexes: 2+ throughout bilat

Repeated Movement Testing:

  • C-T extension over chairback had no effect on baselines
  • Sustained mid-TH ext over a ball had no effect on baselines
  • Ext in lying had no effect

I sent her out with sustained TH extension over a ball.


TAKEAWAY: Despite no significant changes during the session, her history and exam indicate that a derangement needs to be ruled out. Sustained positioning into extension is what I went with again, given her history of worsening with static positioning. Sometimes you will not get that "Aha!" moment, and that's OK, fall back on your history.
